Consistent, Actionable, Fast, and Trustworthy: COVID-19 Messages as a Ringtone on Cellphones

Early in the pandemic of COVID-19, when fear and misinformation spread as quickly as the virus itself, authorities faced an urgent challenge: how to deliver health messages that were accurate, timely, and trusted? To meet this acute need, Pakistan responded with an innovative solution, harnessing the power of 170 million registered cellphone users. Instead of traditional dial tones, the government transformed everyday ringtones into life-saving tools. Each time someone placed a call, a 20–30 seconds recorded message played, sharing up-to-date COVID-19 information and clear preventive actions. Eleven distinct messages were broadcast in the first year alone. The reach was extraordinary: spot tests showed 74 million people recalled these messages, while serial surveys revealed that responding to the messages delivered through a media-mix, 68–81% washed hands regularly, 76–84% wore masks, and 48–62% practiced social distancing during the first year. This low-cost, high-impact intervention demonstrates how simple technology, creatively applied, can shape public behavior at scale—a model for future risk‑communication strategies.
